Today I would like to write a mini post and answer the question - “Why remove wisdom teeth if they do not bother?”, And comment on the statement - “My relatives and friends, dad / mother / grandfather / grandmother / neighbor / cat - they removed a tooth and that went wrong. Absolutely everyone had complications and now there are no deletions.” To begin with, I would like to say that the complications did not arise from the fact of tooth extraction, but from how this extraction was carried out. This could happen for several reasons:
- During the deletion, something went wrong, and it was carried out incorrectly.
For example, there might be a fragment of a root that could not be removed. Sometimes it really happens that a piece of the root is broken, you can’t get it out. The doctor decides not to torment the patient anymore, so as not to cause even more injury. Well, or not to hurt the mandibular nerve, which runs very close to the roots of the 8th lower teeth, trying to get this piece out of there. You ask - "How so?" And so. If there was no acute, and even worse, purulent inflammation, and a small, motionless fragment of the root remains in the hole, then nothing terrible will happen, it will simply overgrow. Naturally, I am not saying that you should leave broken roots in the holes without trying to remove them. But if the doctor understands that “picking” can only do harm, this is not the worst decision. I repeat if did not have acute inflammation, otherwise the tooth must be removed completely, as it is infected.
- Sterility was not observed during manipulation.
I'm not even talking about the processing of instruments and their sterilization, which in every medical institution should be perfect. It’s elementary that a doctor may not wash his hands, having already put on gloves, grab onto something, a phone, a computer mouse, a bag of a patient who asked for it, there are a lot of options, and then with these hands into your mouth. Nobody canceled asepsis and antisepsis.
- The patient ignored the recommendations given by the doctor.
If all that I spoke about above, of course, can happen, although I think there are not many such doctors. At least I hope so. That some time ago, when I worked in a clinic that was located in a “sleeping” area of the capital, it was not so rare for patients to come who did not follow the recommendations at all.
And they went to the bath - “How can you not? I have been going for 20 years! Stable, every week!”
And they were actively involved in sports - “How could I quit training, I’m getting ready for the Olympics!”,
And as if they didn’t want to hear what to rinse nothing is IMPOSSIBLE! - “After the removal, I came / la home and immediately rinsed / with a decoction of medicinal herbs, chamomile, oak bark, and infusions on deer antlers to disinfect. It was recommended to me by a neighbor."
Even an elementary unauthorized refusal of medications prescribed by a doctor can lead to a huge number of complications. Ask what drugs? Classically, no one wants to drink antibiotics. Although there are those who, at the first sensation of soreness in the throat, or the onset of a runny nose, throw antibiotics like candy, not realizing that this is not an antiviral drug. And they don't want to. An antibiotic is prescribed for a reason, but in order to avoid complications. The area where the 8th teeth are located is especially prone to inflammation and suppuration. This means that taking antibiotics after the removal of impacted 8 teeth is mandatory. If, of course, you want to risk and acquire a peripharyngeal abscess, like one of my patients who decided to ignore the recommendations, then you are welcome.
So what am I talking about… Oh yeah. Does not hurt!
Here is a good example of what the untimely removal of a wisdom tooth can lead to. And it doesn't hurt! The man came with a completely different problem, they discovered it by chance when they took a panoramic x-ray of the teeth.
Due to the incorrect position of the 8ki on the contact surface of tooth number 7, a rather deep carious cavity was formed, extending deep under the gum.
The wisdom tooth was successfully removed, but the seven is next in line ... (8 is divided into three fragments - the crown part and two roots)
It seemed to be a normal tooth. “Well, caries, there, there is only one filling, put another one, it’s business!”. Everything is not so simple, since the carious cavity goes deep under the gum, such teeth cannot be treated. Why? Because when placing a filling, the treated cavity must be dry. It is impossible to achieve this with such a defeat. At least due to the fact that the gum contains “gingival fluid”, which will constantly leak into this area.
What to do? Option one is tooth extraction and implantation. Alas.

What do you think the patient asked for? No, not with wild pain or swelling, as many might think. And here's what - "Chet, my food is clogged from the bottom right, look." That is, the young man is only concerned about the food clogging ... just food clogged, Carl! To the question, did it hurt? The answer is “No, it never hurt and nothing bothered.” Well ... you already know the procedure in this case. So much for you - "I'll wait until it bothers you."
If you think that this is the worst thing that can expect you, no matter how it is.
These cysts (and this is not the largest yet) can grow in your jaw and not bother you in any way. Naturally, the tooth must be removed. In order to avoid an increase in this neoplasm, and the occurrence of possible complications. Before this, the channels in the adjacent 7th tooth must be treated, since its roots are in the lumen of the cyst.
Problem solved. The patient is happy. But all this could have been avoided by going to the dentist for a scheduled check-up.
This is the picture that awaited us a year after the removal. Everything dragged on. Everything is fine!
And this could threaten both a fracture of the jaw and damage to the mandibular nerve, which is accompanied by numbness of the lips and chin from the side of the causative tooth, and this numbness can remain for life.
The problem is that many do not go to the doctors, even when it hurts. Although, I don’t think that this can be attributed to Habr users. But for a certain category of people it is quite difficult to convey that “does not bother”, not an indicator that everything is in order.
Questions like, “I have an 8ka curve, but do I need to delete it?” I will answer right away. Wisdom teeth need to be removed almost always! All these "almost always" I have already described How are these teeth removed? . Especially when the 8s cut through incorrectly or did not cut through at all.
If all your wisdom teeth “crawled out”, do not rush to rejoice and think that everything is fine. It may seem to you that they are in the correct position, but do not be lazy and go to the dentist, this may turn out to be a delusion.
