Anthony Newman said: “Every aspect of the game has been upgraded to some level, including the audio. I don’t know if you noticed, but after running, when Ellie stops, her breathing quickens.” The game director then explained why this happens and how it affects gameplay: “Out of sight, what happens is that [Ellie] has a fluctuating heart rate. It increases in melee combat, when running, the presence of nearby enemies and taking damage. The fluctuations in heartbeat carry a wide range of breathing sounds that the main character will give out.

However, this mechanic is not limited to Ellie. All enemies, including clickers, have a heart rate. This parameter affects the behavior of enemies, for example, the infected will become more noisy, which will allow more efficient planning of routes on the ground.
