\nSo, theoretically you can do it as you imagine, but... it\u2019s difficult.<\/p>\n

Of course, there are \u201cmasters\u201d who actively practice and promote this method. But none of them will guarantee that after one or even two years of trying to extract such an 8th tooth and align it with the others will be 100% successful. There are also techniques for tooth replantation, which I am quite skeptical about. Especially in cases where an artificial \u201csocket\u201d (just a \u201cdimple\u201d in the bone) is drilled out where the 6th or 7th tooth has been removed for a long time, into which a horizontally positioned wisdom tooth is also placed. This tooth, in turn, needs to be treated endodontically (that is, its nerve needs to be removed). Don\u2019t you think that\u2019s absurd?<\/p>\n

In my opinion, it's simply foolish, but! Such things do happen. Everyone \u201cworks\u201d in their own way, as they wish or are capable of. As they say, \u201ceverything depends on the indications.\u201d I express my point of view, which may differ radically from the opinions of others.<\/p>\n

So why not extract the wisdom tooth?<\/p>\n

Orthodontists place braces, move teeth, extract \u201clying\u201d retained (erupted) canines that are positioned horizontally in the jaw. So let\u2019s also \u201cextract\u201d the 8th tooth! You might say.<\/p>\n

The problem is that the area of wisdom teeth, especially the lower<\/b> 8ths is quite specific. The bone tissue in this area is very dense, and the site is usually wide. This area is even used as a donor site during bone grafting operations.<\/p>\n

\nThat is, in this location, with the help of special instruments, a bone fragment (block) can be taken and transplanted to areas where there is not enough bone tissue for implant placement. And this area (where the bone fragment was taken) will regenerate over time, making it possible to repeat the procedure if necessary.<\/p>\n

\nBut bone grafting is a topic for separate articles, which we will definitely discuss later.<\/p>\n

So, the bone is dense and wide. If you try to extract the 8th tooth, a deep bony pocket will form behind it, and any self-respecting tooth should be surrounded by bone tissue on all sides. A small example \u2014 take a stick and stick it into the sand, then move it: what happens? A 'trench' will appear in the sand. The same issue will occur in the bone. Extracting a horizontal tooth, ensuring it is surrounded by bone on all sides, is extremely doubtful.<\/p>\n

\nYou might ask, 'Well, what about a vertical tooth instead?'<\/p>\n

I would respond that the situation with a vertically positioned tooth is somewhat different; such significant movements won't be necessary. However, the problem remains the same: moving the 'body' of the tooth is quite challenging. We all know that as a person ages, healing processes in the body occur more slowly compared to younger individuals, such as with fractures. This is due to the fact that children's bones contain significantly more organic substances than those of adults. The outer layer covering the bone (the periosteum) is thick and well-supplied with blood. And so on. The older a person becomes, the longer and more complicated the restorative processes. The same story applies to teeth. If you're around 14 years old, all the tooth movements planned by the orthodontist will occur much faster and easier compared to if you were 40. The same applies to the 'extraction' of canines I mentioned earlier \u2014 if this is done at the age of 14, the likelihood of success for this procedure is maximized.<\/p>\n

\nOn the other hand, if at around 40 you take your first panoramic dental X-ray and the doctor discovers a horizontally positioned retained canine, the chances of success are much lower. Similarly, if you were 14, such a manipulation could theoretically be possible, and I can even assume it would be successful. But there's a big BUT! At that age, the roots of the 8s are still not fully formed; on the panoramic image, we can only see the formed crown part of the tooth, located in the follicle (the capsule surrounding the tooth germ). So, what would you be 'pulling'?<\/p>\n

\nIn such a case, you could damage the tooth germ, and the tooth would still have to be removed. Yes, and if at 14 you had to extract one of your teeth... that's putting it mildly \u2014 sad. So, what will happen to the teeth by the age of 40?<\/p>\n

And one more point, not particularly important but worth mentioning. This is the anatomy of the crown's form and size of the 7th and 8th teeth. They differ. It is possible to establish a full contact in this case, but would it be correct?<\/p>\n

\"If the 6th tooth has long been removed, can the 7th shift into the place of the 6th, and the 8th into the place of the 7th?\"<\/p>\n

No\u2026 It would look something like this \u2014 \"Wisdom

\n\"A holy place is never empty.\" In the long absence of any tooth, neighboring teeth gradually start to shift toward that space. Such movements only occur forward. That is, if the 8th tooth is removed<\/a><\/noindex>, the 7th tooth will not tilt back as shown in the picture. If there are no bite issues (tooth alignment problems).<\/p>\n

\"Can I remove only the lower wisdom tooth and leave the upper one (or vice versa), since it doesn\u2019t bother me?\"<\/p>\n

Unfortunately, no.<\/p>\n

Below, there is indeed an example not involving the 8th tooth, but the meaning is the same. In the absence of any tooth, its antagonist (the tooth that makes contact with it) begins to gradually extend toward the missing one, \"trying\" to find contact.<\/p>\n

\nPlacing an implant in the area of the 7th tooth is not a problem, but properly prosthetizing (placing a crown on) such a tooth will be impossible. Why? Because in this case, the crown will be twice as low in height. This creates what is known as a \"block\" during the movement of the lower jaw, which I mentioned. in this article<\/a><\/noindex>.<\/p>\n

A logical question \u2014 \"What should be done then? How to handle this situation?\"<\/p>\n

Here\u2019s the thing. We call upon our beloved orthodontists and, using special devices and springs, we strive to position teeth correctly, as nature intended. I truly believe that orthodontists are the most important dentists. Why? If you think about it, all dental problems stem from one issue \u2014 their positioning. If teeth are 'crooked,' food debris accumulates more readily between them, which harms hygiene, leads to cavities, and all the complications that arise from that. Plus, the overload on teeth due to improper occlusion. Hello, wear, chips on teeth, and those wedge-shaped defects (non-caries damage that occurs at the necks of teeth in the form of a wedge-shaped defect). The TMJ (temporomandibular joint) also suffers; you may experience crunching, clicking, pain, etc. If there are no bite issues, you can just brush your teeth and you'll be happy. But as funny as it sounds, it has to be done correctly. You could scrub your teeth for 20 minutes, but it wouldn\u2019t do any good.<\/p>\n

I got sidetracked. Here\u2019s a small clinical case.<\/p>\n

\nAn implant was placed, and orthodontic treatment was started simultaneously. As we can see, the lower right 7th tooth is tilted, and the upper right 6th tooth has somewhat intruded downward.<\/p>\n

Note that it is not necessary to install a complete brace system to solve this problem. It is enough to attach 3 brackets to the 4th, 5th, and 7th teeth, and with the help of a special spring, push the problematic tooth into place. The situation is somewhat different in the upper jaw. To resolve the issue, two orthodontic screws are installed. One from the cheek side, and the other from the palatal side. Two buttons are attached to the teeth, and traction (special rubber bands) is applied. They are the ones that 'pull' the tooth into place.<\/p>\n

\nNow my question is, why do you need this? I'm talking about extracting the 8th tooth.<\/p>\n

A wisdom tooth is not a 'spare tire.' You can\u2019t just replace a lost tooth with it. Besides the fact that the process of movement is very long, especially with age, it's also not guaranteed. In other words \u2014 you spent about a year, maybe two, on 'pulling' the 8th tooth. No one will give you any guarantees, and in the end, it may happen that you still have it removed. Is it worth it?<\/p>\n

It is indeed possible to place a single implant in the area of a missing tooth, and after about 3 months (if we are talking about the lower jaw), you will have a fully functional chewing tooth that can last you a lifetime. No more \"let's wait and see.\" All of this, provided you follow all recommendations and visit the dentist every six months for preventive check-ups. With an implant, nothing will just happen on its own. You might ask: - \"Then why come in?\" To see if issues arise with adjacent teeth, as they can affect the implant too. Whether it's a problem with the gums or the bone surrounding it. Preventive checks with mandatory X-rays of the teeth will help avoid such issues. And, of course, professional oral hygiene every six months is ideal, especially for people with bad habits, like smoking. There's no need to explain why; it's clear to everyone.<\/p>\n

You might say, - \"That's disproportionately expensive!\" or \"Natural teeth are better!\"<\/p>\n

Regarding cost. I don't want to disappoint you, but the surgical phase, plus the installation of the orthodontic device and the replacement of bands over a couple of years with an orthodontist, will ultimately be almost comparable in cost to the placement of an implant and the creation of a crown. However, in the first case, there are no guarantees, while in the second case, it's a lifetime guarantee. Do you feel the difference?<\/p>\n

Natural teeth are definitely better. Always. You should fight for them to the last. But this is only the case if those teeth are important. And certainly, wisdom teeth do not fall into that category, as they only bring problems.<\/p>\n
