We have already discussed a lot about wisdom teeth, what types exist<\/a><\/noindex>, how they are removed<\/a><\/noindex>, not hurting does not mean everything is fine<\/a><\/noindex>, there's nothing to do in CHL<\/a><\/noindex> and even more so to 'pull them out'<\/a><\/noindex>. I am very pleased that many of you liked the articles, but today \u2014 I will continue the topic of implantation.<\/p>\n

We all know that our people visit doctors only in exceptional cases. When it\u2019s already too late. A visit to the dentist is no exception. Of course, this is not much related to the users of Habr, but I would like to share and, most importantly, show you how this can end.
\n<\/a><\/noindex>
\nSo, let's get started!<\/p>\n

What are people so afraid of? What stops them? Everyone has their own reason. When it comes to dentistry, in my opinion, there are mainly two fears: the fear that it will hurt (or even more than it does now) and the fear that it will be expensive. People think it's better to spend that money on a vacation, a new car, or\u2026 8PACK OrionX. Priorities vary for everyone.<\/p>\n

But few realize that delaying a visit to the doctor can only exacerbate the situation. Often, while you think 'I\u2019ll endure and it will pass', the situation can worsen to the point of serious complications, where the only option is to call an ambulance. However, it also happens that many dental problems are asymptomatic and are discovered only by chance. Thus, \u2018not hurting is fine\u2019, later leads to the situation where none of the teeth can be saved and all have to be removed. As we know, the larger the volume, the more difficult the task and the higher the cost. This applies regardless of the field. Therefore, it is extremely important to visit a dentist every six months to monitor all those 'not hurting' situations in a timely manner. Why six months? It is believed that within no more than six months, a problem can be detected and eliminated at its initial stage.<\/p>\n

Here is one example<\/p>\n

\nThe patient is quite sensitive about her teeth. As we can see, she has actively engaged in the treatment and restoration of her teeth. However, time passes, which means the lifespan of fillings, crowns, and bridges has come to an end. Beyond her own decaying teeth, problems may also arise with the installed implants, as in this case. The second ones also have to be removed. Not to mention that some doctors still place plate implants without any indications. They can break very easily, as is the case here. And why is that? Because there was no comprehensive approach, treatment plan, or clear vision of the situation. Tell me, why was a thin plate inserted here with such a bone width? Surely, the conditions were even better before the operation. At least not worse, for sure.<\/p>\n

\nThe only right solution will be to remove the plate implant. Although... removal is a mild term. It will have to be sawed out. What do I mean? I mean, YOU HAVE TO SAW IT OUT. After that word, somewhere on the horizon, the doll Billy on his bicycle starts to slowly but surely approach, and the ability to adequately perceive information gradually disappears, would you agree?<\/p>\n

\nAs we know, plate implants lack integration. This means they do not fuse or bond with the bone. They only hold in place mechanically. When forming the bed for the implant, a \"trench\" is made along the alveolar ridge, into which this plate is placed. Over time, bone tissue grows through the holes of this implant, creating something akin to a lock. Therefore, it cannot be removed in any way other than what I mentioned above. You might ask, isn't the removal of a typical cylindrical implant done in the same manner? Okay, now compare the wound area when removing a plate, which is about 2 cm long, to that of a cylinder, averaging 4.5 mm in diameter. Is there a difference? Moreover, if for some reason, problems arise with the cylindrical implant, it is usually either not integrated (not fused with the bone), and thus can be removed with fingers, or there has been a critical loss of bone tissue around the implant, as in this case. Often, the work of the drill or ultrasonic tip is minimized, just like the trauma after the procedure. However, this does not diminish the difficulties associated with restoring the volume of lost bone in this area, as there is usually a significant \"hole\" left behind.<\/p>\n

\nSo, after taking into account the results of the diagnostics, the consultation with the orthopedic specialist, and, importantly, the patient's preferences (!), a decision was made to remove all teeth on the upper and lower jaws, including previously installed implants. Aside from the plate implant, I left that one for dessert.<\/p>\n

Do you think that's all? Can we start? Not so fast! At this stage, new fears begin to arise, such as \u2014 \"What?! Remove them all at once?!\", \"Will I even survive?\", \"How will I chew with my gums later?\"<\/p>\n

\nIn fact, there is nothing dangerous about this. Your health, and even more so your life, is not at risk. Moreover, no one will let you leave the clinic without teeth. Before extraction, impressions of the jaws are mandatory, and subsequently, complete removable dentures are made by a technician in the laboratory. After the work arrives at the clinic, the patient is scheduled for tooth extractions, followed immediately by fittings and the delivery of the temporary prostheses. This means that just as you arrived at the clinic with teeth, you will leave with teeth.<\/p>\n

\nBefore proceeding with the implantation, about 2 months should pass for complete wound healing. There\u2019s no point in waiting longer, as the bone will not regenerate, and its volume will increasingly decrease over time. The jaw will not dissolve completely, but with prolonged tooth absence, and consequently, a lack of load in that area, the bone tissue will slowly begin to diminish. The longer you delay restoration, the worse the conditions will be at the time of implantation. This means the likelihood and necessity for bone grafting will only increase.<\/p>\n

Well, two months have passed, and it's time to start the implantation! But how can we place implants if there are no teeth? What should we base the placement on, to ensure they stand straight and in their proper place? We can't just place them haphazardly. To avoid this:<\/p>\n

\nA surgical template is used. A special cap that resembles a sports mouthguard, with one condition: holes are drilled in it for the teeth that will be implanted later. This is necessary so that the surgeon can understand where exactly the implant should be placed. Below is the positioning template, which serves only for marking:<\/p>\n

\nIn the case of this patient, a separate surgical template is not required. The orthopedic surgeon will create similar holes in the temporary prosthesis using a drill, which will serve as the template. After the operation, the same doctor will seal these holes with a special material, and the prosthesis can continue to be used until the permanent construction is made. And no, putting it in a glass of water before bedtime will not be necessary.<\/p>\n

In the panoramic image below, the contrasting 'white cylinders' are clearly visible in the middle; this is the very material that was used to close the holes in the upper removable prosthesis. The prosthesis itself is not radiopaque, so it is not visible in the image.<\/p>\n

\nAnd for dessert. Behold! Here it is, CREATIVITY! This is what I was talking about, a plate implant with holes in it, through which bone tissue has grown. And the broken 'pin' that was one of the supports for the bridge prosthesis.<\/p>\n

\nWhat about the other support, you may ask? Drumroll, please. Your own teeth! The canine and first premolar (tooth 4). The patient brought a photograph. It's quite old, film and not the clearest, but it is what it is. (I took a photo with my phone).<\/p>\n

\nSome might think, what's the big deal? Well, an implant, well, a tooth. A bridge and a bridge. But teeth have a ligament apparatus, one of the functions of which is cushioning. That is, when chewing, the teeth have a bit of 'spring' to them, while the implant is rigidly fixed in the bone and lacks this function. It turns into something resembling a lever. This causes an overload in the transition area of the 'pin' into the body of the implant, resulting in its fracture.<\/p>\n

So, shall we summarize!<\/p>\n

Dear friends, you must understand that neither the large volume of work, nor the removal of all teeth, nor bone grafting, nor the number of implants installed is frightening. What is truly frightening is that one small \"I'll endure it\" can turn into a big \"I should have done this yesterday.\" The longer you wait, the more extensive and prolonged your treatment will become. By taking care of your teeth in a timely manner, you can prevent cavities. Treating cavities at early stages will help you avoid complications such as pulpitis or periodontitis. If you treat pulpitis or periodontitis promptly, you can avoid tooth extraction. Timely restoration of a lost tooth will protect you from bone grafts and so on. After all this, it seems pointless to say that timely visits to the dentist, or any other doctor, will shield you from unnecessary stress and expenses. It's all clear without words. So, brush your teeth, stay fit, and let's meet more often for preventive check-ups rather than when dental issues arise.<\/p>\n
