I'm going to do something I don't think I've ever done before, which is to use my full authority as the person who discovered and wrote down the rules of open source.
After ten years of drama and idiocy, many people, including myself, are now willing to publicly declare that "codes of conduct" have been a disaster - a kind of contagious social madness that breeds drama, political intrigue, gossip, and negative productive work.
Here's my advice on codes of conduct:
Abandon it. If your project has such a code, remove it. The only real function it serves is as a tool in the hands of those who like to add fuel to the fire. If you are forced to have such a code for bureaucratic reasons, replace it with the following sentence or something similar: "If you cause more trouble than your contributions justify, you will be removed." Attempts to make codes more specific and detailed are ineffective. They only give provocateurs the opportunity to manipulate. Yes, we must strive to be kind to each other. But we must be ruthless and merciless toward those who try to weaponize "Let's be kinder!" Indulging them never ends well.
